Prevention of Homelessness across the West London Boroughs
We provide one-to-one support to young people aged between 13 and 19 who are experiencing difficulties with their housing. This could include sleeping rough, being evicted from your home, thinking of leaving home and don’t know what your choices are, or living in temporary accommodation and needing advice and support.
The service has created a video aimed at raising young peoples’ awareness of leaving home and the problems they can encounter such as not being eligible for assistance with their housing. This video is available for schools and agencies to use as a tool to discuss the issues of homelessness with groups of young people. Video coming soon...

Mediation Services
LINK Mediation
The objective of our Mediation Service is to work with young people who are at risk of becoming homeless from their parental home because of a breakdown in their relationship with their parent or carer. Lara Rose meets the young person and their family with the view to rebuilding the relationship or find some practical way forward.

LINK Workshops
Kate De Rees, Education and Outreach Officer, provides one off and sets of workshops for young people - be it at school, referral unit, or in a youth club. She tells them what youth homelessness is really like and helps them learn ways to improve family life. If a young person is having problems at home with their family she will signpost them to LINK Mediation for one to one support.

The Move On Project
Young people are placed in Hammersmith & Fulham Young Person's Assessment Centre by Cleo Lewis for approximately 6 weeks. Once placed in the Assessment Centre they are allocated a floating support worker who works with a young person to improve their life skills and encourage them to live independently. Cleo will then collate and review a young person's needs and find them suitable supported accommodation to move on to.
